Is there a form I could fill to force my wife to let me see the children?

Three months ago she kicked me out of the house (which is in her name) and is preventing me from communicating with, or seeing, the children. If I try to go she says she'll call the police - that I'm tresping. We both want to divorce but I'm willing to wait the statutory year apart before filing for divorce whereas she wants me "out of her life right away" and won't wait. She wants me to sign a separation contract she had her attorney write up - something to the effect that everything in the universe is my fault - and I wouldn't sign it because it made no provisions for seeing the children without her around, nor could I afford an attorney. It would have paved the way for our divorce but now she thinks I'm dragging my feet. She wants to expedite the divorce proceedings and is using the children to that end. She said You wanna see your children again? YOU take ME to court. I still can't afford a lawyer, but if a form exists, that could be filed with the proper authorities that would enable me to see my children again, I'm there! IS there such a form, or means, to this end? Thank you.
